Sanctuary Personnel, an innovative and committed recruitment agency, has a new permanent position available for a Specialist Senior Social Worker within the Transition Team to work full time based in Hull.
The salary for this permanent Senior Social Worker job is up to £38,296 per annum.
Main duties:
1. To promote and safeguard the welfare of children, young people and/or vulnerable adults.
2. Conduct skills active listening and confidently facilitate difficult conversations to ensure people’s hopes and wishes are efficiently and timely planned and supported. Actively promotes independence and wellbeing in the transition process from the age of 14, working closely with young people and families directly. Support the development of staff/team to promote an approach to people who use services that is based on the principles of co-production.
3. To promote and develop integrated and collaborative ways of working to ensure that the best possible outcomes are achieved.
4. Engages in complex cases that require autonomous decision making, multi-agency/disciplinary co-ordination and or legal preparation and planning.
5. Coordinates services and people in relation to a transition of care and support for young people moving into adult services. To support families and carers to adjust to changing circumstances. To oversee delivery of personalised non-intrusive and seamless proportionate and timely intervention.
6. Contributes to team planning, effectiveness and responsiveness. Supports team manager with improving practice and performance or individuals/team by providing day-to-day education and supervision.
7. As directed prepares and attends decision making forums including supporting co-workers with decision relating to complex/high risk cases.
8. Ensures that case recording is accurate, up to date, in line with legislation and is available for a variety of purposes e.g. multi-agency, meetings, strategy meetings, panels, reviews, court.
9. Supports Line Manager to review working practices to deliver a continuously improving responsive service that recognises individual need, strengths and resilience, communicates and encourages innovation, motivation, understanding and vision.
Requirements of this role:
1. A social Work Qualification or equivalent.
2. Understanding of relevant legislation.
3. Registered or eligible for registration.
4. This post requires registration with the relevant health professional council/governing body (Social Care England).
