Health Referral Advisor Based at: Hybrid/Requirement to be at Frodsham Leisure Centre on a Friday Hours of work: 37 hours per week – Part time job share considered Contract type: Permanent Salary: Grade 5 - £25,183 per annum Closing date: Thursday 23rd January 2025 Interview date: Proposed date of Friday 31st January 2025 Job Purpose: As a Health Referral Advisor, you will support Brio's mission to improve community health and wellbeing in alignment with Cheshire West and Chester Council’s ‘Play Your Part Towards a Stronger Future’ Plan. Through our Integrated Wellbeing programme, you will empower individuals to make positive changes, reduce health inequalities, and enhance their quality of life. You will be part of a small team who provide high-quality person-centred care, triage direction, client advice and administration for Brio’s Wellbeing programmes. You will support all clients either directly referred or self-referring into our Integrated Wellbeing programmes, and will co-ordinate their participation in the relevant programme, supporting them in various ways throughout their behaviour change journey as they complete their programme. Main Duties & Responsibilities: Client Support Work within a team of Health Referral Advisors serving as the first point of contact for individuals referred to Brio’s Wellbeing programmes, including smoking cessation, falls prevention, weight management, long COVID recovery, and pulmonary rehabilitation. Provide telephone appointment booking support which includes completion of a full holistic needs assessment; triaging them into the relevant Brio Wellbeing programme, ensuring each person meets the inclusion/exclusion criteria. Provide compassionate, person-centered support via phone or email to a diverse age range of clients and guide them through their health journeys. Support clients who access our programmes to access other services and organisations as necessary to address wider determinants of health and wellbeing. Refer clients who are not eligible for our services on to other relevant health and social care / behaviour change services as appropriate. Programme Coordination Work in partnership with the wider Brio Wellbeing and Leisure teams to coordinate the booking and delivery of our wellbeing programmes including room booking and delivery staff scheduling. Collaborate with local partners and referral agencies (GPs, practice nurses and secondary care) to strengthen referral pathways. And ensure all inappropriate referrals are communicated effectively in a positive manor which strengthens partner relationships. Guide clients to suitable ongoing support options post-programme. This may include online resources, support groups or other local services and activities. Promotion of our programmes at a wide range of community events or workplaces, platforming our programmes and successes to those who need them the most. Administration Maintain accurate and secure records in client referral databases (DCRS and ReferAll) for monitoring and evaluation purposes, ensuring compliance with data protection standards. Contact clients by telephone and process correspondence outlining chosen venue, date & time of appointment. Act as point of contact for the clients and referral agencies. Manage email and telephone messages deciding upon appropriate course of action. Support for ongoing performance reporting including activity and outcome targets and KPIs and by using wellbeing assessment tools with clients at the initial assessment and end of the programme as well as follow ups at both 6 and 12 months after programme completion. Be responsible for ensuring rigorous and regular data inputting to ensure accurate records are maintained in accordance with procedures, to ensure compliance with information Governance and Data Protection. To attend and provide updates on referrals and clients at meetings either face to face or virtually. Seek the views of client’s participants in our wellbeing services using service evaluation questionnaires. Other Support safeguarding processes by implementing procedures, maintaining training, and promoting best practices.
