This post offersan exciting opportunity for a health care assistant to join the Specialist CoronaryHeart Disease & Heart Failure Team based at the Greenhouse in AnnfieldPlain. The post covers core hours of 09:00-17:00 Monday-Friday with currentlyno weekend working although that may change in the future.
Main duties of the job
The successful applicant will be caring for patients withcardiac conditions such as coronary heart disease (CHD) and heart failure (HF).They will be expected to support theHeart Failure Specialist Nurses team in providing care for these patients byrecording observations such as heart rate, BP and pulse oximetry andphlebotomy.
They may be directly involved in delivering cardiac rehabilitation,to both CHD and HF patients, alongside the exercise facilitators andcardiovascular nurses.
Care is delivered in the patients own home andin clinic / community venues around Derwentside

We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.

Essential
1. Able to travel independently around County Durham & Darlington.

4. Experience of working within a healthcare environment.

Qualifications

7. Care certificate or willingness to complete in the first six months of employment.
8. NVQ3 or relevant equivalent experience

12. Competent in performing and recording clinical observations and understand their significance: BP, heart rate and rhythm, pulse, oximetry, and, respiratory rate.

14. Phlebotomy or willing to work towards.
15. Ability to work independently and act accordingly in an emergency situation.
