Job Summary
The Community Children's Nursing Service is looking to appoint an experienced and motivated individual to lead and develop the established Children's Hospital at Home service. This is a fantastic opportunity to work in a highly innovative, unique team gaining experience in service development, Quality Improvement and working closely across community and acute services. The Children's Hospital at Home service, established in 2021, offers specific clinical care pathways to support children receiving care in their own homes. The Team Lead Nurse will provide clinical leadership for acute nursing care, collaborating closely with Paediatricians and the Matron in monitoring the safety and effectiveness of the service.
This role is going through job evaluation so there may be some slight changes in the duties.
Main duties of the job
The CH@H Team Lead Nurse will operate independently, demonstrating clinical proficiency in the advanced assessment of acutely ill children. The role requires completion of advanced practice modules and would be suitable for Children's Advanced Nurse Practitioners looking to develop in a combined clinical and leadership role or experienced children's nurses with a background in recent acute and community paediatric nursing.

Job Description
The children's community nursing team serves children from birth to 18 years across Oxfordshire. The Hospital at Home service accepts direct referrals from GPs, ED and inpatient wards. We support early discharges for assessment and monitoring and provide care for acutely unwell children in the community. We operate from four locality bases across Oxfordshire delivering various services, including caseload management, respite sessions, palliative care, acute nursing interventions, and special school nursing support.
This is a vital leadership position working alongside a countywide leadership team within the Children's Community Nursing Team. The role will assist with service delivery by participating in the end-of-life on-call rota and the manager on-call rota as required. Full training and support will be provided to fulfil this role. This role is based at a locality base, but travel to other areas is required as we offer a county-wide service. The position is for 37.5 hours per week however part time hours would be considered at a minimum of 30hrs/week. The shift pattern will be a variety of early (8am-4pm) and late shifts (12pm-8pm) including weekends and bank holidays.

* Minimum 2 years experience as a band 6 nurse in acute/community setting

* Demonstrates a clear understanding of professional accountability and code of practice
* Can evidence and ability to facilitate, critically analyse and implement research and audit processes
