Job overview
The purpose of the post is to maintain and improve all aspects of patient safety including governance of relevant processes and measures. The role is based at St Bernard’s Hospital.
The post holder will be responsible for the management of the incident review process including ensuring the timeliness and transparency of all review processes and reports and lesson-learning. The post holder will monitor all Serious Untoward Incident (SUI) investigations. They will ensure all reports are of a high standard and that learning lessons is embedded into the service. They will provide clinical oversight along with senior colleagues of review processes. The post holder will lead on the implementation of the Patient Incident Response Framework (PSIRF) in West London Forensic Services.
Main duties of the job
The post holder will ensure quality assurance of reporting internally and externally of the serious incident review process as well as the administration of interlinked databases and operational aspects of the process as outlined in Trust and national policies.
The post holder will ensure incident review facilitation as appropriate to skill/knowledge base and service need and provide a direct link from West London Forensic Services (WLFS) to the Trust Governance arrangements with regard to all issues of patient safety. The post holder will be present and visible in services on the St Bernard’s site. They will be familiar with a human factors approach to support patient safety.
The post holder will provide highly visible on site leadership in relation to the service user and carer complaints process and will manage this in collaboration with colleagues.
The post holder will provide reciprocal leave cover for the functions above for the High Secure Service as needed. The post holder will line manage the Patient Safety Coordinator.

* Evidence of ongoing professional development.

* Educated to Degree level (or equivalent).
* Masters Degree or equivalent in Specialist Area.
