About the Role: As a Trusted Assessor, you will be at the heart of our Care and Wellbeing Service, working in close collaboration with Sheffield City Council. Your mission will be to promote ongoing enablement, helping individuals receiving care to reach their full potential for independent living. You'll play a vital role in ensuring that care plans are dynamic, continually evolving to meet changing needs and preferences. Key Responsibilities: Conduct comprehensive assessments and continuous reviews of care needs. Support providers in fulfilling their duties under CQC regulations. Identify and implement the least intrusive services to boost independence. Proactively address life challenges to maintain quality of life. Collaborate with Sheffield City Council, local Primary and Community Health Care networks, and Community Services. What We're Looking For: To be considered for this role, you must have: A good understanding of the current legislative framework in reference to the Care Act 2014, including eligibility, MCA, and safeguarding. Knowledge of CQC regulations. The ability to recommend changes to care plans and understand the responsibilities and tasks associated with procuring services. A good basic level of education. Customer service NVQ Level 3. A valid UK driving licence and your own transport. The current right to work in the UK. Willingness to participate as a secondary back-up for our out-of-office service on a rota basis. Why Join Us?
