The care coordinator is responsible for the continuity of care for all of our service users. This vital role has a huge impact on the lives of many people who rely on our care and support.
Service Users tell us that what really matters to them is having a stable team of caring and professional care workers whom they can rely upon.
Working alongside the Registered Manager, Deputy Manager, Care Supervisors, and Compliance Consultant, the Care Coordinator will be responsible for making this happen.
The ability to problem solve and think clearly when under pressure is essential for this important role in our agency.
You will use your excellent planning, organisational, and IT skills to make sure that each service user receives the care and support that they expect on time. This involves:
1. Allocating care workers with the right skills and experience to deliver the best quality care and support to each service user.
2. Organising care worker rotas to minimise changes to the service and travel between service users' homes.
3. Responding efficiently to day-to-day changes in the care and support packages.
4. Monitoring attendance to calls within and out of the Electronic Call Monitoring System employed.
