The main responsibilities of the post-holder will be to monitor incoming emails, signposting to the relevant department, monitor and update the palliative care register, monitor care plan recalls, taking appropriate action, read code relevant information from patients' clinical records, hospital correspondence, and other sources, into the practice computer system, working strictly in accordance with the Lister House administrative protocols. The successful candidate will be responsible for inputting confidential and sensitive information that may be triggering for some individuals.
Main duties of the job
* To be an active member of the Data Administration Team, undertaking ongoing training and development in document management and note summary of the patient medical record.
* Ensure inbound correspondence, by whatever method of receipt is managed and flowed through to the appropriate recipient for action.
* To scan patient correspondence to the Practice's clinical system and index to the correctly identified patient record.
* Read code relevant information into the Practice Clinical System (SystmOne) and ensure follow-up actions are identified and completed in a timely manner.
* To keep coding and data input up to date, liaising with the Administration Manager and Senior Administrator.
* To summarise existing patient records, in accordance with the Note Summarising Protocol.
* Monitor the Practice generic email, taking appropriate action in accordance with the set Protocol.
* Run reports and audits on the clinical system to ensure accurate data is provided.
* Monitor task lists, taking appropriate action.
* Collect and process incoming post, distributing to the appropriate staff.
* Take professional pride in the quality of your work and contribute to a positive and supportive team.
* To attend and contribute to staff meetings and development meetings constructively.

* Qualified to GCSE standard or NVQ equivalent.
Experience
