Conduct systematic and comprehensive assessments of client needs, develop care plans within the CPA framework, and facilitate carer support plans. Develop and review risk management plans, identifying opportunities for positive risk-taking in collaboration with clients, ward teams, and carers. Work with clients and their families on medication management, symptom management, and relapse prevention. Coordinate and implement care plans, evaluate their effectiveness, and work closely with community services and multidisciplinary teams to support client recovery.
