Would you like to be part of pioneering service expanding the Out of Hours provision for specialist palliative care services supporting patients across West Suffolk and Thetford? As part of the West Suffolk Alliance, we are committed to supporting people to die well, and where possible in accordance with the choices they have made. This means enhancing the support we provide during OOH periods, whilst working in partnership with our Alliance colleagues. We are looking for a committed Hospice Care Assistant with community experience to work alongside a Specialist Nurse/Paramedic Out of Hours.
Main duties of the job
You will be able to support patients where they call home and educating family carers in their home (Clinical support is provided out of hours). Working alongside the Early Intervention Team within West Suffolk Hospital out of hours to facilitate an effective response to those with unmet palliative care needs. You will also have a close working relationship with St Nicholas Hospice Care colleagues.
