Job summary As a Registered Nurse you will be responsible for supporting your Manager in ensuring that our service users needs are met through the delivery of high-quality care.To provide quality care to patients that require an urgent community response, enabling them to be cared for in their own home thereby reducing avoidable admissions and supporting timely discharge from the acute and community bed setting.The post holder will provide strong clinical leadership to a team of Out of Hours staff ensuring that the appropriate care is delivered to meet the individual needs in the community setting.To provide skilled nursing advice to support and care for patient's in their own homes.You will be responsible for providing clinical and managerial leadership to the Out of Hours Team. You will be required to complete Personal Development reviews, sickness reviews and the completion of rota's to ensure that the appropriate number of skilled staff are available to work. Interview date - 12.03.2025 Main duties of the job To provide high level care to a patient with complex health care needs and their family in a crisis situation. This may include accessing support from the wider Multi-Disciplinary Team and community services to ensure the appropriate treatment and is available to allow the patient remain in their own home. To work in the transfer of care hub to triage patient's onto the appropriate pathway utilising your assessment and clinical skills for effective clinical triage. Utilise effective Clinical triage to support admission avoidance, the 2 hourly urgent response target and utilise the 'step up/down' model across the Virtual ward and Urgent Community Response pathways. To promote a clinical environment that supports good team working. To act as a mentor and preceptor to support the development of individuals staff members training. To utilise a range of communication methods. This will include electronic systems to share accurate, relevant, timely information with colleagues and other agencies. To ensure high quality care is maintained at all times in the service.
