We are looking for an Advanced Clinical Practitioner to join our East Kent HCP Frailty/Home Treatment Service. You will will apply advanced clinical expertise, levels of judgement, discretion and decision making to the provision of clinical care to older people within a variety of healthcare settings. S/he will exercise a high degree of autonomy through the application of advanced clinical skills in relation to the assessment, diagnosis, treatment and management of the older person. You will lead and monitor improvements to quality of care through clinical audit, evidence based practice, clinical supervision, teaching and supporting professional colleagues and the provision of skilled professional leadership. You will lead the implementation of relevant clinical guidelines into practice and contribute to the further development of the evidence base through research and audit. You will lead the development of non-medical led models of care across Emergency departments, inpatient wards, intermediate care, care homes and community teams in collaboration with multi-professional colleagues. You will play a crucial role in improving patient flow through timely assessment, referral, responsive intervention, anticipatory care planning and optimal discharge coordination across seven days Rated ‘outstanding’ by the CQC, we’ve got a passion for delivering high standards of patient care and excellent services to improve the health of our communities.
