Job Description
Qualified Social Worker, Enfield Council, Adults Home Treatment & Hospital Discharge Team - Must be a Qualified Social Worker with a minimum of 2 years+ UK Social Worker experience within an Adult Social Work team.
The team works extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager.
The responsibilities will include:
1. Experienced qualified Social Worker required for a hospital discharge team to joint work with discharge nurse (health colleagues) to assess for patients discharges under the Care Act 2014.
2. Undertaking comprehensive social care assessments of needs including Human Rights, Mental Capacity Assessment or other appropriate reports in line with adult social care remit, with detailed support plans to facilitate patients’ safe discharges.
3. Social Worker will need to have sound knowledge around adult legislation and the continuing health care process and the legislation that underpins this, including the CHC Checklist, Decision Support Tools, London Health Needs Tool.
4. Social worker may also need to communicate closely and sensitively with family members of service users who are deceased.
5. Social worker will need to use a database to record information, access and load documents - Knowledge and Skills on IT systems are essential.
6. Liaise with other professionals.
7. 36 hours per week (flexible by negotiation).
8. 2 Month Temporary Contract.
9. Start date subject to DBS and reference clearance.

The successful candidate will need to hold a DipSW or equivalent, must be registered with the HCPC, have a current DBS, and be a car driver.
