Gtd healthcare are the providers of all minor illness and minor injury care at Preston Urgent Care Centre and minor illness at Chorley Urgent Care Centre. The services provide the first point of access for all self-referred unscheduled attendances for the Emergency department (ED). To facilitate the provision of accurate healthcare information, health improvement advice or, where applicable, referral to the appropriate level and location of care, to meet the holistic and therapeutic requirements of the patient / carer. Main duties of the job
1. Assess and prioritise patients' clinical needs utilising Manchester Triage System.
2. Supplies and administers PGD medications in a safe effective manner in accordance with the appropriate policy and guidance.
3. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
4. Works within their respective Code of Professional Conduct.

Key duties and responsibilities include:
Service Delivery and Performance
1. Act as a professional role model at all times.
2. Develop and maintain expertise, practice and competence in the role of Clinical Assessor. Provide skilled and effective Face to Face / telephone / Video assessment for patients passed from a range of services such as 111 / work as part of multidisciplinary team within the urgent care / primary care / OOHs setting.
3. The Clinical Practitioner will undertake a telephone / video consultation using the computer software system and will always use professional judgement.
4. To apply critical analysis along with a high degree of clinical curiosity.
5. Empower patients to enable them to act upon advice given and embrace self-management.
6. Accurately document consultation notes in a timely fashion to allow for suitable emergency referral and intervention.
7. To share and utilise areas of specialist practice with peers and colleagues to inform and enhance patient care.
8. To work with the management / operational team to contribute to the development and delivery of service.
9. Identify and use information sources such as the Directory of Services (DOS) to support and underpin clinical decision-making.
10. Provide health education and self-management advice during the consultation process.
11. Refer to the ACP Lead / Lead SCP / medical Lead as necessary for advice.
12. Facilitate and actively participate in the continuous quality improvement audit process. Reflect as necessary on this process.
13. To attend and contribute to organised meetings.
14. To follow gtd healthcare's policies and procedures, including the failed encounter procedure and the home visiting policy.
15. Understand and embrace local pathways including use of ED for urgent and immediate needs.

5. Utilise advanced listening skills, probing and facilitative skills across a diverse range of calls and face-to-face assessments, some of which are highly challenging due to emotive circumstances or patient aggression.
6. The post holder should recognise the importance of effective communication within the team and will strive to communicate effectively with team members, patients, and carers.
7. Utilises enhanced communication skills to negotiate and provide support to service users who may be non-compliant with recommended outcomes whilst adhering to the relevant gtd healthcare policies and procedures.

Have responsibility and accountability for the escalation of any adverse incidents or events to the appropriate persons both internally and externally, whilst ensuring that the appropriate documentation is recorded, and that the ACP lead / operational lead is actively involved and communicated with.

3. Provide health education and promote healthy lifestyle for patients and carers.
4. Provide clinical and professional advice.
5. Promote safe systems of work and identify areas where changes in Urgent Care / primary care would achieve a more effective outcome.
6. Ensure resources are used efficiently and effectively and all equipment used in the care of patients is appropriately maintained and used only by those individuals with the appropriate training.
