Job summary
We have an exciting opportunity for a motivated and enthusiastic Health Visitor SCPHN to work within our 0-19 Growing Healthy Service in Stockton on Tees.
This post will be based from Billingham Family Hub and the team you will be aligned to are a skill mixed team covering a geographical area of Stockton. We have designated teams for infant feeding, healthy weight and SEND which support us to collectively improve the health outcomes for children and young people living in Stockton on Tees.
We work in partnership with Midwives, Early Help, Children's Services, Public Health and Acute Services.

Main duties of the job
New team members are supported through induction, training, shadowing and a preceptorship package tailored to your individual needs.
1. 4-6 weekly supervision with Line Manager and Safeguarding Team

Deliver the commissioned service with a focus on safety and quality.
We are an innovative team dedicated to promoting the health and wellbeing of the Stockton on Tees population.
The Health Visitors role is to utilise caseload profiling, identify the health needs of a designated area and promote programmes for the family/population to alleviate the problems caused by physical, social and emotional ill health.
Liaise closely with other health professionals, statutory and voluntary agencies in order to ensure a cohesive plan of care for individuals, families and communities is delivered.
Health Visitors plan, organise and deliver programmes of care in conjunction with the family and the multi-disciplinary team and undertake continuous on-going assessment of children, young people and families. They provide an evidenced based service to children, people, families and communities in accordance with National and Local Policy initiatives.
The post holder will need to work as an autonomous practitioner, assuming professional and legal accountability for all aspects of own work in line with the Trusts objectives and national and local guidance.
You will need excellent communication skills and a passion for public health, alongside good organisational skills, flexibility to meet the needs of the service, and ability to travel independently to meet service delivery as required.
Ensure complete, accurate and contemporaneous records in accordance with the NMC and Trust guidelines, policies and procedures.
There is a robust package of clinical and safeguarding supervision provided to all staff.
