Inpatient Pathway Occupational Therapist

Come and make a difference by joining our innovative Intermediate Care Reablement Team. The role of Occupational Therapy is essential in supporting people to transition back to their home environment, by focusing on what they want to, need to, have to, and most importantly LIKE to do to ensure their lives have purpose, value, and meaning.
The team consists of a Discharge Co-ordinator, OTs, Physios, Reablement Support Workers, and Adult Social Care Workers, all working with a shared vision of putting the person at the center of their care. The role is based within a care home team within walking distance from the center of Taunton, and the ethos is to enable people to be as independent as possible. We see people not only for their ongoing rehabilitation and support needs once discharged from the Acute hospital but also through to the next stage of their journey into their home environment. This role works in collaboration with ongoing services to enable a person to maximize their potential in all aspects of occupations that are important to them.
Due to progression, we have a fantastic Band 6 full-time opportunity. We have flexible working arrangements if required to support you with your work-life balance and competing needs.
Main Duties of the Job
The post holder will be required to work their hours over 7 days, including out of hours, weekends, and bank holidays, to meet the needs of the service. A UK driving licence, business insurance, and access to a car for work duties are essential.
In this post, you will be supported to:
1. Provide high-quality patient-centered care and specialist rehabilitation to adults referred to the Intermediate Care Bedded Pathway Service.
2. Develop, use, and be creative with your OT skills to work holistically and look deeper than the original diagnosis.
3. Have the freedom and support to act in patients' best interests.
4. Work with patients to set personalized goals and individual treatment programmes.
5. Work with patients, their families, and the MDT to plan and implement discharge.
6. Clinically supervise other members of the team.
7. Connect with, assess, and treat patients who may have diverse presentations and complex physical and psychological conditions.
8. Take a role in the development of the Intermediate Care Service within Somerset.
Your professional supervision includes support from the team leader, who is an experienced AHP therapist, working with you in intermediate care. Links with other OTs within the intermediate care service will be supported.
The Occupational Therapy leads and Occupational Therapists across all agencies in Somerset are working together to implement our OT strategy with the aims of better experiences for staff, better experiences of care, better outcomes, and values.
