An exciting opportunity has arisen for a motivated and enthusiastic Advanced Clinical Practitioner (ACP) to work in a busy, older adults community mental health setting, within the Memory Service. This role involves the assessment, diagnosis and treatment of diagnostic and functional mental health conditions. The post holder will be expected to exercise a high degree of autonomy and make critical judgments, and work flexibly as part of the ACP team.

Main duties of the job
The role of the Advanced Clinical Practitioner (ACP) is identified as a clinical expert and is required to have a high clinical profile ensuring the provision of effective and efficient care. At this level, the post holder is expected to work as a highly skilled senior practitioner and to have expert knowledge with the ability to deliver autonomous and competent practice in all four domains of the ACP framework. You will be expected to confidently lead on complex and challenging cases and support the team in a creative and committed approach.

Job responsibilities
The job description for this post is generic to Advanced Clinical Practitioners and maps out the role of the fully qualified Advanced Clinical Practitioner. However, it is important to note that as services are progressing through the phases of transformation, the roles and responsibilities of the Advanced Clinical Practitioner will be shaped during the implementation to reflect the evolving needs of the service and as such are not confined to one objective area. There will be an expectation of flexibility to be able to work across both functional and organic illness, adults, and older adults, with support into areas of less familiarity, with no diagnostic exclusions.
The Memory Service ACP role is varied and interesting with a high level of autonomy that encompasses the assessment, diagnosis, and treatment of both organic and functional mental health problems, using the full range of medical, psychological, and social opportunities available. The clinicians within the teams require a wide-ranging skill set to support service users with a broad range of complex mental and physical health problems, including, but not limited to, personality disorder, psychosis, eating disorders, co-occurring substance use, and anxiety/depression.
Person Specification
Qualifications
* Registered healthcare professional (e.g., NMC / HCPC or multi-professional equivalent)
* Non-Medical Prescriber qualification / or multi-professional equivalent (or willing to work towards)
* 2 years post qualification experience at Band 6
* Relevant degree (second class or above)
* Experience in applying principles of adult and children safeguarding / Mental Capacity Act / Mental Health Act
* Understanding of clinical governance, audit, and quality assurance
* Demonstrable practice at a minimum of Band 6 in the field of mental health
* Must hold current driving licence
* Evidence of higher level of practice development at a senior level within specialist area
* Post graduate experience/training at a senior level or qualifications in one or more areas of the four pillars of advanced clinical practice
* Demonstrable evidence of leading on policy development, management of change and audit of quality
Experience
* Previous experience of working within, or demonstrable knowledge of one of the areas of specialty:
* Long term conditions
* Complex Dementia
* Primary Care Networks (PCNs)
* Personality disorder
* Rehabilitation MH
* Co-occurring substance use
* Demonstrable experience of autonomous decision making and of leading multi-disciplinary meetings
* Previous experience of delivering teaching or training
* Previous mentor or supervisory role and ability to demonstrate principles and values of clinical supervision
* Significant experience where a degree of autonomous practice has been required
* Commitment to anti-discriminatory practice with experience of multi-professional working
* Understanding of the current climate in relation to evolving PCN networks and implementation of FYFV
* Ability to audit and interpret NICE guidance and formulate and implement recommendations
* Demonstrable and extensive understanding and knowledge of all age, mental and physical health conditions
* Previous management or leadership roles or qualifications and responsibility for clinical / managerial supervision
* Have led in complex situations and cases where adult / children safeguarding / use of the mental health act / use of the mental capacity act has been a feature.
* Key role in service development in relation to FYFV and PCNs
* Evidence of previous NICE guidance baseline assessment and implementation of same
Knowledge
* Leadership and change management skills, evidence of leading projects of change
* Effective communicator with service users, relatives, and all members of the multi-disciplinary health care team involved in primary and secondary care
* Ability to work in a calm and efficient manner in situations of significant workload with the ability to manage own caseload and effectively assess risk
* Knowledge and understanding of cultural and equal opportunities issues
* Comprehensive understanding of care planning principles / Crisis/ Contingency and risk management planning
* Demonstrable evidence of positive influencer practice in complex multi-disciplinary care with key decision making and skilled chairing of MDT meetings / case discussions and reviews where risk is a significant consideration
* Demonstrable motivational skills within depth knowledge of change management and individual learning styles
