We are seeking an experienced Practice Nurse with a minimumof one year experience working in Primary Care, and a qualification in diabetesor a keen interest in developing expertise in this area. The successfulcandidate will play a crucial role in managing and supporting patients withdiabetes, contributing to their overall health and well-being. They will workas a key part of the clinical team and must be able to organise and managetheir own workload.
Main duties of the job
* Treatment room duties to include venepuncture, wound care, administration of injections and immunisations, carrying out ECGs, removal of sutures and staples, advice on ear care and carrying out ear irrigation where appropriate, urine sampling.
* Support and supervise trainees with treatment room duties in accordance with their competencies.
* Carry out cervical screening where postholder holds a current smear takers code.
* Provide reviews for patients with long term conditions based on QOF. This may include diabetes, hypertension, cardiovascular disease, chronic kidney disease, learning disability and mental health reviews.
* Provide high-quality nursing care and support to patients with diabetes.
* Conduct diabetes clinics, including assessments, education, and management plans with the support of the Diabetes Nurse Lead and GP Lead.
* Collaborate with GPs and other Healthcare professionals to deliver integrated care.
* Participate in health promotion activities and patient education. Integrate health promotion and patient education within annual reviews.
* Maintain accurate and up-to-date patient records using EMIS.

A current team of 5 partners, 2 salaried GPs, a paramedicpractitioner, nurses, and HCAs, all supported by an accredited Practice Managerand a full admin team.
Other team members include and Health Coach,physiotherapists and Clinical Pharmacist.
We are a teaching Practice which includes GP Registrars,Medical Students and Apprentice HCAs.
A team involved in training and research projects andcommitted to our local PCN.

Job responsibilities
This role is responsible for working towards our annual campaigns and ensuring we are meeting the required Quality Outcome Framework (QOF) targets for chronic diseases. In addition, you will be expected to take part in the extended hours rota for the Practice.

Person Specification
Qualifications
* GCSE grade A to C in English and Maths
* Registered Nurse with valid NMC registration.
* A minimum of 1 year experience in Primary Care
* Knowledge of the needs of patients with long term conditions
* Aware of accountability of own role and other roles in nurse led services
* Awareness of clinical governance issues in Primary Care
* Ability to support patients to change lifestyle.
* Excellent communication and interpersonal skills.
* Ability to work independently and as part of a multidisciplinary team.
* Effective organisational and time management skills
* Qualification in diabetes or a strong interest in gaining expertise in this area.
