Salary: £27,000 – £36,000 per annum pro rata (dependent on skills and experience)
Working commitment: 0.6% FTE/ 21 hours p/w (potential to increase to full-time, funding dependent)
Contract: Fixed-term for 12 months (potential for extension to at least three years, funding dependent)
Location: Working in community settings in Manchester with some home working.

About the Role:
* We are looking for an experienced Independent Living Outreach Worker to join our Ageing Well in Place Partnership team.
* You will be carrying out one-to-one case work with older and vulnerable people living in high rise social housing in the neighbourhood of Hulme, Manchester.
* You will be working together with tenant committees and an Ageing Well in Place Development Worker to increase service access, household income, and overall wellbeing, as well as improve integration and effectiveness among local providers.
About you:
* You have good interpersonal skills and a proactive, systematic approach to achieving positive outcomes for older or vulnerable adults.
* You are experienced in enhancing the ability of older and/or vulnerable people to live well and with independence in the place they call home.
* You will have a passion for confidence and capacity building and for working with diverse people to achieve improved levels of wellbeing.
