Job summary We have an exciting opportunity for an experienced Learning Disabilities Nurse. Working in the community as part of a multi-disciplinary and multi-agency team, the post holder will provide specialist nursing interventions to adults with a learning disability. The KCHFT learning disability team uses a positive multi-disciplinary model to provide direct specialist clinical expertise, behavioural and engagement support and access to developmental opportunities to people with learning disabilities and their families and carers. We also facilitate reasonable adjustments to enable people to access mainstream healthcare services where this best meets their health needs. The multi-disciplinary teams of Occupational Therapists, LD Nurses, Speech & Language therapists, Physiotherapists and a specialist Vision and Hearing service work in five geographic localities across the County andwork in an alliance with the social care teams for Kent County Council (KCC) and the LD teams within the Mental Health Trust (KMPT) to provide a co-ordinated and holistic combined service for people with learning disabilities. Main duties of the job As a LD Nurse you will assess, plan, implement and evaluate holistic care linked to packages of prescribed care. You will support in the supervision and development of all junior staff including Health Care Support Workers and Students. You will ensure that resources are used effectively, planning workload to meet the priorities of patient care by the most efficient use of time, equipment and manpower and other resources (medicines, medical devices) In addition you will undertake a range of competent clinical interventions to support the outcomes of both individual and group therapy.
