Job summary
Secondment opportunity 12 months (maternity leave cover). An exciting opportunity has arisen to join the Doncaster Community Stroke Rehabilitation Team which comprises of Occupational Therapists, Physiotherapists, Nursing staff and Rehabilitation Assistants. This service covers the Early Supported Discharge and Community Rehabilitation Service.
The Rehabilitation Assistant will support the qualified professionals and contribute to the assessment, implementation and delivery of care within patient's homes.
Main duties of the job
Undertake agreed activities with patients/service users under the direct supervision of the qualified staff. This will include:
1. Provision of mobility equipment
2. Supervision of patients whilst completing exercise programmes
3. To report progress of the patient and changes in condition to the appropriate person.
4. To ensure accurate documentation of care within the electronic record keeping programme
5. To develop clinical competency and skills through the completion of competency based training, and to maintain skills through regular re-assessment and training
6. To demonstrate sensitivity to the patient and carer's needs by respecting privacy and dignity, whilst maintaining professional boundaries at all times.
7. Develop a rapport with patients/service users and encourage them to participate in the rehabilitation/healthcare programmes and activities
8. To work using own initiative within the community with minimal supervision following a period of supervised practice.

Qualifications Essential
* Written, verbal and numeric skills to GCSE grade C or similar
* NVQ - level 2
* Evidence of learning from reflection
* Basic internet and technology skills
Experience Essential
* Have previously worked with people in a health care setting.
* Have worked within a Multidisciplinary team
