Care Management Assistant
We're excited to collaborate with a Local Authority in the Bromley area who are seeking a Care Management Assistant for their Adults Services.
Offering a 6 month agency contract with potential for ongoing extension, this full-time position (36 hours) is based in BR1.
The competitive rate of pay is £17 - £21 per hour, negotiable based on experience.

About the Role:
1. We are looking for an experienced Care Manager Assistant to work closely with the Senior Social Worker to avoid Social Admissions to Hospital.
2. Supporting Carers by completing urgent assessments and support plans to prevent Cares breakdown.
3. Supporting Vulnerable adults ensuring they have the appropriate support to meet their assessed needs.
4. Undertake assessments of Adults and Carers needs in accordance with Care Act 2014, and procedures.
5. Prepare individual support plans reflecting the assessment of needs of the Service User and their Carers including identifying unmet needs.
6. Arrange for the provision of standard adaptive equipment and demonstrate the correct technique for use ensuring that the Service User and Carers are safe and independent as possible.
7. Request minor adaptations to the Service User’s home in accordance with service procedures and check that these are suitable after installation and the Service User and Carer is as safe and independent as possible.
Experience Needed;
8. Care Act 2014.
9. Facilitating Hospital discharges.
10. Hospital Admissions.
11. Supporting Younger Adults with physical disabilities.
12. Local Authority / Public Sector.
