Midhurst Macmillan specialist palliative care service is looking for a dynamic nurse band 5 Community Nurse ,who is passionate about patient experience and the delivery of quality, patient focused care .
You will be working as part of a multi-dispciplinary team to provide comprehensive palliative nursing care, advise and support to patients and carers living in the community across three Clinical Commissioning Groups and a geographical patch of approximately 400 square miles.
This position involves collaborating with Community Nurses, Primary Care Teams, other Health Care Professionals and Agencies to meet complex patients care needs
Communication is a key part of the role, and you will be required to manage challenging conversations with sensitivity and understanding
You will need to seek consent from patients and their carers to share information with other Health Care Professionals

1. Key responsibilities
· You will be required to use Trust documents, Instruction Charts and Care Plans in planning patient care
· You will be required to understand key End of Life issues and trends
Main Tasks
· To be responsible for the delivery of care and advice, with the support of the Team Lead and other Health Care Professionals to individuals in the Community
· To be responsible for prioritising your own workload
· To include first assessment when appropriate, on-going assessment, development, implementation and evaluation of individualised programmes of care in the Community
· To work in a variety of settings including patients’ own homes, Nursing Homes, Residential Homes, Community Hospitals and Inpatients’ Clinic
· To provide support and guidance to patients, carers and relatives and to teach appropriate skills to provide on-going care
· To care at times, for dying patients, ensuring that the patient and their significant others are given all relevant information as appropriate and in a professional manner
· To support and move patients in compliance with moving and handling policy and with the use of appropriate equipment
· To participate in the setting of professional standards, quality assurance and clinical audit programmes to ensure we continue to deliver high standards of care
· To participate in training and induction programmes for all members of the clinical support team
· To be responsible for ensuring own personal and professional development and updating of skills and knowledge. This may include a range of Clinical Skills such as Venepuncture, IV Administration, IV Cannulation and management of Syringe Drivers
· To be responsible for ordering appropriate community equipment for individuals to allow them to remain at home and to undertake relevant training under the Sussex Community NHS Trust guidelines
· To understand the economics of the service
· To order stock in a cost-effective way
· To participate in and attend all relevant meetings and committees and disseminate information as appropriate
· To be aware of and act in accordance with Sussex Community NHS Trust Policies and Guidelines and the NMC Code of Conduct
· To fully participate in the Trust Appraisal and Supervision programme and plan personal development on an annual basis
· To undertake training as necessary in line with the development of the post and as agreed with line manager and to undergo all aspects of staff induction as soon as possible after appointment to meet with mandatory and statutory requirements
· To undertake duties at the request of the line manager which are consummate with the role, including project work, internal job rotation and absence cover
