We are looking for a highly experienced and committed senior nurse to lead the Planned Care Unit. The postholder will play a pivotal role in managing and coordinating nursing activities to ensure the highest standards of patient care. You will lead a team of dedicated and dynamic nurses, collaborate with multidisciplinary teams, and contribute to the development and implementation of care plans.
The Planned Care Unit, based in Willow Unit, has been an established specialist service to accommodate day treatment and medical procedures on a planned basis. Among these are biologic and magnesium infusions as well as blood transfusions. Medical procedures like abdominal paracentesis, lumbar puncture, and venesections are just a few that are performed in this clinic. It is a nurse-led service aimed at delivering excellent nursing care through admission avoidance and early supportive discharge.

Main duties of the job
1. The post holder will be responsible and accountable for the delivery of health care to patients within the Planned Care Unit in Acute Medicine.
2. The post holder will have continuing responsibility and accountability for the assessment of patient care needs, the development, implementation, and evaluation of care, and the setting of standards of care.
3. The post holder will manage the deployment and supervision of staff and the teaching of students, orchestrating the timely and appropriate admissions of patients to the Trust.
4. The post holder will actively support change, service development, and the rollout of new initiatives to promote patient-centered, personal, and effective care for all.
5. The post holder will undertake the leadership and development of an identified group of nurses.
6. The post holder is responsible for hiring, evaluating, and managing nursing staff, as well as developing and implementing improved nursing processes and procedures.
7. The post holder focuses on measuring patient outcomes, overseeing patient transitions of care, assessing risks, and building communication channels between teams.
8. The balance between practice and management will vary according to day-to-day priorities but will be negotiated overall with the Medical Matron.

Job responsibilities
1. To act as the lead nurse of the service and nurse in charge of the day in coordinating patients attending for their treatment and procedures on a daily basis.
2. To take and exercise clinical judgement in accepting suitable referrals from medicine, surgery, haematology, other specialties, and General Practice according to agreed pathways and criteria.
3. To process and action email referrals received daily.
4. To work collaboratively with different clinicians and their junior doctors in the management and delivery of patient care.

Qualifications and knowledge
* Registered nurse (adult) with valid NMC registration
* Master's degree (Level 7) in a relevant field or equivalent experience
* Teaching and assessing qualification
* Evidence of continuous professional development

* Undertaken Advanced History Taking and Patient Clinical Physical Examination course Level 7 or equivalent
