Main duties of the job
1. Provide clinical expertise, making decisions about the best course of action for patients' care (both in bed-based rehabilitation and within the community setting). This will involve assessing patients and providing personalised rehabilitation plans, whilst working alongside multiple agencies, to determine the best management plan for patients.
2. To provide leadership and clinical supervision to junior staff members.
3. To challenge multi-disciplinary team discharge decisions to ensure all patients in cluster beds meet the Criteria to Reside.
4. Deliver programmes of supported patient rehabilitation, in ways that facilitate behavioural change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, and that ensure patients can be maintained safely within their own homes, if possible.
5. To maintain and develop links with the wider multi-disciplinary team to support and improve community rehabilitation pathways across East Cheshire.
6. To use advanced clinical skills and clinical reasoning to provide comprehensive assessment and treatment support to patients who present with complex conditions and/or multiple pathologies and/or mental health and/or pain management needs.
7. To use advanced physiotherapeutic handling skills for patients with a variety of complex needs. These handling skills will need to be performed consistently and accurately across a range of different conditions e.g. falls, balance, manual handling, respiratory and neuro.
8. To use highly developed physical skills in manual handling to assist patients, their families/carers and wider members of the MDT with complex moving and handling plans for patients with equipment needs e.g. hoisting, standing equipment, facilitated transfers.
9. To participate in developing education and training for specialist therapists to support the development of community rehabilitation and share knowledge within the wider therapy workforce.
10. To provide training and supervision, to junior staff and students.
11. To provide and receive clinical supervision and be an active provider of mentoring and coaching of specialist clinicians from a range of disciplines.
12. To provide leadership and support on community rehabilitation and service development across the care communities, alongside learning opportunities for the whole multi-disciplinary team within ECNHST.

Qualifications
* HCPC registration
* Relevant professional degree/diploma/MSc pre-registration
* Clinical Educator training
Knowledge
* Post registration knowledge which is relevant to the specialist area
* In-depth knowledge of recent NHS legislation, recommendations for specialist area
* Detailed awareness of the national and local priorities and changing trends in the specialist area
