Main duties of the job You will need to be able to work in an autonomous way and facilitate effective communication between ward staff, the integrated discharge team and social care team, to promote a culture where transfers of care between primary, community and secondary services are safe, timely and well co-ordinated for patients who no longer require acute hospital care. Part of the role will involve working within the Community nursing teams proactively supporting and case managing a caseload of complex patients with a range of long term conditions, who are at high risk of unplanned admission to hospital or prone to unnecessarily long lengths of stay. You must be able to travel throughout the working day across the borough.
