Job summary This role is based the West District Nurse Community Team Location: Acomb The role of the Clinical Coordinator is to be a point of contact for the team, handling all referrals and tasks made by our single point of access team, to triage these tasks and allocate to the appropriate staff in the team. You will require excellent communication skills as you will speak to patients, carers, GPs and other members of the multi-disciplinary team. You will require the skills to handle a variety of conversations with patients, sometimes during times when patients feel upset or anxious. The clinical coordinator will require good knowledge of SystmOne and patient care needs. Initial training will be provided and ongoing support will be provided by the team leaders. The working pattern will be flexible within the current shift patterns, to meet the needs of the service. It is expected that this role will predominately be Monday - Friday, however there is an expectation that the successful candidate is prepared to work all shifts across the day and 7 days per week if required.
