Job summary A fantastic opportunity has arisen for a highly specialist B7 nurse to join the existing heart failure team in central area. The successful post holder will be required to manage complex heart failure patients; a key aim is to keep patients well at home and to avoid hospital admission where possible. The new service will be fully integrated with other service providers to ensure care is provided at the right level to meet patient needs. You will be responsible for managing your own patient caseload of complex heart failure patients which will mainly be in a community setting but will also require input into secondary care to ensure case identification and onward referral to the community heart failure service. You will be required to have advanced skills such as clinical history and examination non medical prescribing; clinical management of these patients will be delivered to a high standard. You will also support the team in delivering training and education in all care settings to other HCP and to patients and carers. Excellent clinical and communication skills are essential requirements for this post. Main duties of the job To develop and sustain a service to improve the health and well-being of individuals and their carers who suffer from Heart Failure. To be able to act autonomously in the assessment and management of Heart Failure patients using protocols, policies, clinical supervision and clinical judgement to guide practice. Work in partnership with staff across BCUHB and Primary care to ensure patients with heart failure are managed appropriately and to act as a specialist resource to other health care professionals. Provide education and training to healthcare professionals about heart failure, ensuring all information disseminated is evidence based, thus facilitating a rise in overall standards of care across BCUHB. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
