Main duties of the job The integrated discharge hub based at Milton Keynes Hospital, is responsible for being a single point of contact for professionals, providers, patients and their families to support safe and timely discharge from hospital. The ethos of the hub is to provide a seven day service to ensure patients are central to all discharge planning and they, along with their families are kept updated through their discharge journey and seamlessly returned to their home/chosen residential setting. The hub is responsible for the safe and timely discharge of patients, reducing delays in discharge by coordinating professionals to ensure the necessary support and services are in place.
