To provide operational day to day management and leadership to the major trauma rehabilitation team Within the MTC to work with the trauma coordinators, Rehabilitation Medicine (RM) Consultant, therapy teams and other medical and nursing staff to support the patients discharged home. To liaise with health professionals in other parts of the patient pathway including community therapy teams, GPs, District Nurses and voluntary and independent sector organisations as needed.To liaise with and support patients families and carers. To coordinate and lead the trauma rehabilitation team multidisciplinary team meeting. To coordinate the rehabilitation care pathway of patients following serious or major trauma admitted to Major Trauma Rehabilitation ward who need ongoing rehabilitation as a result of their injuries, working as part of the multi-disciplinary trauma team. To liaise with Therapy, Nursing and Rehabilitation Medicine staff facilitating engagement in the rehabilitation prescription process, by identifying and coordinating patients for review. To provide specialist advice to support assessment, planning and implementation of care, as part of the team responsible for programmes of rehabilitation for patients who have sustained Major Trauma and serious trauma. To pro-actively facilitate transfer to an appropriate rehabilitation setting in a timely manner, where the prescribed programme of care can be delivered. Accountable for own professional actions, not directly supervised. To have specialist knowledge and expertise in the rehabilitation of patients who have sustained major and serious trauma, including the rehabilitation of patients with musculoskeletal trauma, head injury, spinal cord injury and other neurological injuries. To provide specialist education and training to other staff and learners. To ensure data and outcome measures for all patients is recorded and used appropriately to improve patient care and support service development To carry a clinical caseload of the most complex patientjourney through the clinical pathway, providing continuity and advocacy of the patients rehabilitation needs as they move from one clinical setting to the next. To work with the trauma coordinators and clinical team to identify patients with ISS over 8, who need a rehabilitation prescription and to have input into the Rehabilitation Prescription process with the clinical teams, using knowledge of the patient and their carers and social circumstances to aid assessment of their rehabilitation needs. To provide support for patients with ISS>16 and work with the Rehab Medicine Consultant and clinical teams to identify and address rehabilitation needs in this group. To attend and contribute to (2-3x weekly) RM rounds for this group.
