An exciting role for an Advanced Clinical Practitioner has been created to join the dynamic Community Treatment Team (CTT) that covers Barking & Dagenham, Havering, & Redbridge. The CTT aims to avoid unnecessary admissions to acute hospital settings by managing patients in crisis in their own homes. The post holder will deliver high quality holistic nursing as part of a multidisciplinary team within the accident and emergency and community setting, working with patients towards rehabilitation, self-management and independence throughout the continuum of care.
Main duties of the job
Job Purpose :
The post holder will use advanced assessment and diagnostic skills to assess and treat patients. The post holder will be an autonomous practitioner, collaborating with colleagues in primary care, community and hospital settings as appropriate.
The post holder will:
* Be a Qualified Advanced Nurse Practitioner
* Act as a role model
* Be responsible for undertaking comprehensive health assessments, physical examinations, and ordering tests for patients in Rapid Response
* Use advanced clinical decision making and assessment skills, coordinate care planning, and initiate referrals to diagnostics for appropriate tests for this population.
* Be involved in establishing, delivering, and leading a comprehensive service within community services
* Play a leadership role within the team and work with external stakeholders such as London Ambulance Service and Social Services to develop new ways of working and increase the number of referrals from acute and community settings.
* Lead on service development

Clinical Practice:
* Be professionally and legally accountable for all work undertaken and practice at an advanced level of professional autonomy and accountability within national guidelines and the Nursing and Midwifery Council's Code of Conduct.
* Provide clinical leadership and maintain credibility through practice and professional update.
* Act as an expert nurse and resource for patients, families, and staff.
* Admit, discharge, or transfer patients to and from the service as their needs require.
* Undertake advanced clinical assessments of patients referred to the service to enable prompt assessment, diagnosis, and treatment according to agreed policies, protocols, and guidelines.
* Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
* Order and interpret blood tests and initiate appropriate action as required.
* Provide and request appropriate diagnostic and/or therapeutic interventions for a holistic assessment of the client group.
* Maintain and develop own repertoire of advanced clinical assessment skills.
* Prescribe medication within own level of knowledge and competence and within the scope of the Trust's Policy for Non-Medical Prescribing qualification.
* Facilitate the further development of clinical skills for all nurses caring for patients.
* Ensure adequate protection for self and staff if exposed to body fluids, odours, and infections.
* Complete documentation relating to patient care and ensure that staff maintain and update documentation as changes occur according to Trust policy.
* Liaise and work closely with relevant multi-disciplinary team members, including General Practitioners and therapists in the provision of professional service.
* Prioritise, organise, and manage own workload to maintain and promote quality.
* Contribute to the development of local guidelines, protocols, and standards.

Qualifications

* Recent primary and community nursing experience
* Nurse led management of minor illness, ailments, and injuries
* Good management/project management skills

* Advanced clinical practice skills in managing patients with long-term conditions and acute exacerbations
