Main duties of the job
As a St Lukes Community Nurse, you will play a vital role in providing specialist palliative care. This is a varied, rewarding position that blends clinical excellence with holistic care. Working as part of a multidisciplinary team, you will assess, plan, and deliver tailored care for patients with progressive and life-threatening conditions. You'll support patients in achieving their preferred place of care, help prevent avoidable hospital admissions, and provide compassionate guidance for families and carers. This role offers opportunities to develop specialist skills, support students and junior colleagues, and be involved in education and training. You will work under the clinical leadership of advanced practitioners but will be empowered to act with autonomy and clinical decision-making.

Job responsibilities
1. Provide high-quality, patient-centred palliative care in the community.
2. Undertake holistic patient assessments, create care plans, and manage symptoms in collaboration with the wider multidisciplinary team (MDT).
3. Work with primary care teams to support patients and carers, ensuring informed decision-making and respect for patient choices.
4. Participate in triaging referrals, discharge planning, and coordination of care.
5. Contribute to the education and development of junior colleagues and students.
6. Act as a role model and clinical leader, ensuring compassionate and dignified care is at the heart of every interaction.
7. Support St Luke's commitment to innovation and continuous improvement in palliative care.
Person Specification
Experience
* Registered Nurse with active NMC registration.
* Experience in community nursing, palliative care, or end-of-life care.
* Strong assessment, planning, and decision-making skills.
* Knowledge of disease trajectories, symptom management, and person-centred care.
* Ability to work autonomously, using professional judgment and evidence-based practice.
* Excellent communication skills to support patients, families, and MDT colleagues.
