Main duties of the job Under the guidance of our experienced senior nurses, you will become an integral member of a multidisciplinary team dedicated to providing long-term care within the Transplant Directorate. Key responsibilities will include; -Providing essential support to patients and their carers during inpatient and outpatient assessment, as well as while they await lung and/or heart transplants and mechanical circulatory support devices. You will also manage their care whilst in the community to ensure continuity of support. -Enhance your expertise by educating patients, carers and staff on mediation management and long-term post-transplant care. You will coordinate outpatient services and manage a telephone helpline, contributing directly to improved patient outcomes. -Coordinate and contribute on ward rounds for all transplant inpatients as part of a 7-day rota. Gain valuable experience in donor and recipient coordination through exposure to a 24/7 on-call rota.
