Job summary An exciting opportunity has arisen within our Continuing Health Care Teams across Greater Manchester. We are looking for highly motivated and experienced RMNs, RGNs OR Allied Health Professionals who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments so CHC and FNC eligibility can be determined. Roles involve assessing patients for CHC, holding a caseload, and reviewing patients, as necessary.
