Health Connectors working for the West Mendip Primary Care Network support people to take proactive steps to improve the way they manage their physical health, mental wellbeing and day-to-day lives, based on what matters most to them.
They are experts at working with individuals to help them to tap into and activate support networks, and linking them to practical support. They use health coaching techniques to help people to develop their knowledge, skills and confidence in managing and improving their health and wellbeing.
The West Mendip Primary Care Network are looking to recruit two Health Connectors with a passion for preventative, person-centred healthcare. One will have a particular focus on supporting patients and community engagement in the Glastonbury area, working closely with Glastonbury Health Centre. The other postholder will provide support across West Mendip, and will also have dedicated time to support patients with dementia and cancer.
The Health Connectors will be part of a multidisciplinary team, working closely with health, social care and community professionals. They will have a varied caseload, which will include supporting patients with complex health and social needs.
A full driver’s license and access to a car are essential, as travel across the West Mendip area is required.
