Main duties of the job
The role will involve offering consultations to patients within a practice and supporting the needs of the patients as required.
* Clinical Assessments - the successful candidate must be able to offer clinical assessment for a range of presentations within a standard clinic.
* Treatment Decisions - the successful candidate should be able to independently identify appropriate treatment and, where required, order necessary investigations.
* Prescribing Medication - as part of a consultation, the need to prescribe medication may arise. The successful candidate should have an appropriate qualification in prescribing and show knowledge of the latest prescribing guidelines.
* Referrals - where appropriate, referrals to secondary care may be required as part of the process; appropriate forms may need to be completed prior to referral, and knowledge of local guidelines is desirable.
* Home Visits - as part of the clinical team, attending the home premises of a patient may be necessary to undertake clinical assessments or offer treatment.

Job responsibilities
Key responsibilities (this list is not exhaustive)
* Work within own competence and professional code of conduct to provide consultations to patients in a clinical environment, both face to face and via the telephone where required.
* Produce accurate, complete records of patient consultations, consistent with legislation, policies, and procedures.
* Deliver care according to NSF and NICE guidance.
* Assess effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation.
* Work within the wider team, supporting colleagues in meeting the needs of our patients.
* Participate in the maintenance of quality governance systems and processes across the organisation and its activities.
* Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
* Support and participate in shared learning across the practice and wider organisation.
* Participate in the management, review, and identification of learning from patient complaints, clinical incidents, and near-miss events utilizing a structured framework (e.g., root-cause analysis).
* Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedures and local guidance.
* Work within policies relating to domestic violence, vulnerable adults, substance abuse, and addictive behavior, and refer as appropriate.
* Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.
* Participate in and support local projects as agreed with the practice management team.
* Undertake mandatory and statutory training.
* Apply infection control measures within the practice according to local and national guidelines.
* Use technology as an aid to management in planning, implementation, monitoring, presenting, and communicating information.
* Review and process data using templates and accurate SNOMED/Read codes to ensure easy and accurate retrieval for monitoring and audit processes.

Person Specification
Experience
* NHS experience.
* Experience of working within a nursing team.
* Experience of dealing with vulnerable patients.
* Knowledge of needs of patients with long-term conditions.
* Knowledge of minor illnesses and appropriate treatment.
* Ability to discuss and communicate difficult messages to patients/their family.
* Ability to identify the needs of patients with long-term conditions.
* Knowledge of public health issues in the local area.
* Awareness of local and national health policy.
* Knowledge of PGD/PSD and associated policies.
* Experience of SystmOne.
* Able to offer additional services to patients outside of minor illness clinics (cervical cytology, immunisations/vaccinations, QOF reviews).

Qualifications
