Job summary We are excited to be recruiting a Consultant Practitioner in Frailty to work alongside our existing Consultant Workforce at Urgent Community Response and Frailty HH Virtual Ward. The Team operates across Mid Hampshire to deliver acute hospital level care at home in response to urgent changes in health status for patients living with frailty. This vacancy is a direct result of our success and the recent expansion of the service through increased commissioning and a very successful GIRFT review. The successful applicant will be joining an established and supportive group of Consultant Practitioners from varied backgrounds. The successful applicant will be supported to develop their Multi-Professional Consultant Practice Portfolio working towards the 'Expert' capabilities. Main duties of the job The post holder will be required to work autonomously. The foundation nature of the post means appropriate supervision from Consultant Practitioner is available to support the Foundation Consultant Practitioner to develop and work towards the competencies expected of a full Consultant Practitioner.
