Job summary An opportunity has become available in the Integrated Discharge Hub (IDH) for a permanent Band 4Integrated Health & Social Care Assessor D2A. We need a resilient and dynamic individual to join the experienced and enthusiastic team. This is an ideal opportunity for any individual who has a knowledge of the Care Act 2014, who wants to work within an unplanned care setting and access a variety of training via the social work learning academy (SWLA) You will work collaboratively with other Trust services to provide seamless care for adults/carers on defined care and support pathways with particular emphasis on facilitating the discharge of patents through the Disharge to Access (D2A) model. The work can be both challenging and rewarding and involves liaising with patients, their family's and ward professionals via telephone or face to face as appropriate to ensure gathering of accurate information in order to facilitate effective, safe and timely discharge from the acute hospital. This is a 7 day per week service and you will be required to work shifts patterns which cover 8am-8pm and include weekend on a rota basis. You may also be require travel to cross cover at a number of sites across North Staffordshire. Main duties of the job You will be will be part of a supportive, multi-disciplinary team within an often challenging environment so will need to be able to work at pace and be able to move between competing priorities. You need to be an excellent communicator, have strong written and verbal skills and be able to transfer information accurately onto a Transfer of Care form to identify patient needs and formulate effective person centred outcomes to support effective patient transfer.
