We are looking for a GP to lead a new team at Ranworth PCN. Ranworth PCN is part of the national PCN pilot looking at new ways of working in primary care. We are happy to consider applicants on a full-time or job-share basis and are therefore flexible re the number of sessions to be worked In line with the Fuller report, this role is to lead one of these projects. The proactive ambulatory care model is part of a broader shift within the NHS toward more community-based, patient-centred care that aims to keep individuals out of hospitals and support them in managing their health conditions more effectively at home. The Ranworth PCN Proactive Ambulatory Care Team is a healthcare initiative aimed at improving patient care by providing proactive, out-of-hospital treatment and support, particularly for individuals with complex or long-term health conditions registered with us as housebound or resident in a care or residential home. We have around 900 housebound patients and 450 care home beds. This approach is designed to prevent hospital admissions and reduce the need for emergency care by managing conditions in the community and enhancing patient outcomes. As the Proactive Ambulatory Care Team Lead GP, you will play a crucial role in the planning, delivery, and management of proactive healthcare strategies aimed at improving the health outcomes of patients, specifically for our 450 care home beds and 900 house bound patients. You will lead a multidisciplinary team to enhance patient care through early interventions, preventive care, and health promotion. This position also involves leading quality improvement initiatives and ensuring that patients receive timely and appropriate care in accordance with best practice standards.
