An exciting opportunity has arisen to appoint a highly motivated Health Care Support Worker to join a well-established home oxygen and Non Invasive Ventilation (NIV) service. This role will be working within the Swansea Team.
The post will be based at Singleton Hospital, but you may be required to work at other sites across the Health Board. Patients will be seen in the Out Patients Department, and also at home across Swansea Bay.
You will assist with the review of patients and undertake capillary blood gas measurements, following training and home risk assessments.
You will be working alongside the MDT from primary and secondary care to assist with the holistic assessment of patients and agree on a plan of treatment and care with the patient at the heart of the decision-making process.
The role also requires you to support patients with palliative needs, supporting patients and their families and liaising closely with community palliative care services to ensure continuity of care.
You must be organised, confident, have experience of working as a Health Care Support Worker, and have excellent communication skills to enable you to liaise effectively with the multi-disciplinary team and work with a diverse and varied group of patients and carers.
Experience in Home oxygen assessment or NIV is not essential, but evidence of your wider respiratory knowledge, clinical skills, experience, and a willingness to learn must be demonstrable.
