PRACTICE PHARMACIST
PART-TIME
Due to expansion of the practice, we are recruiting a part-time Pharmacist to join our innovative and progressive practice.
We are a multisite GP Practice with a list size of circa 45,000 patients, operating a triage model of care supported by a multi-disciplinary team including GPs, ANPs, pharmacists, and physiotherapists. We are looking for someone who would like the opportunity to impact the future development of General Practice.
We are a dynamic practice responding creatively to the changing demands of primary care by:
1. Utilising concepts of demand and capacity management
2. Clinical & social integration in hub-based working
3. Providing quick patient access with no appointment backlog
Main areas in this role would encompass but are not limited to:
1. Identify and review patients with medication-related issues (including patients with polypharmacy)
2. Triage and where appropriate, manage acute prescription requests within the practice
3. Ensure a safe, robust, and efficient repeat prescription system within the practice and contribute to the regular medication review process
4. Conduct medications reconciliation for patients discharged from hospital and newly registered patients
5. Deal with outpatient clinical requests
6. Handle queries from community pharmacies, hospital pharmacists, care homes, patients, and carers
7. Supervise other members of the pharmacy team, e.g., Pharmacy Technicians and Pharmacy Clerk
8. Carry out quality improvement activities/audits within the practice
9. Be involved in care home medication management, including conducting medication reviews, helping with processing monthly medication requests, and attending multidisciplinary review meetings
Independent prescribing would be preferable; however, mentorship will be available if wishing to complete the independent prescribing course. Mentorship will also be available to develop skills in telephone consultation and triage, clinical decision-making, and risk assessment.
Our way of working foreshadows many of the planned changes to the way that primary care will be delivered in Scotland. Therefore, joining our practice would be ‘future proofed’.
