Main duties of the job
Clinical
Perform complex holistic wound assessments following referral from healthcare professionals. Plan, implement and evaluate specialist care with patients and carers using diagnostic tests/equipment where appropriate Doppler and full vascular assessments.
Be able to demonstrate/obtain advanced clinical skills such as conducting full vascular assessments including differential diagnosis.
To support triaging of clinical assessments and care planning via telemedicine for patients as appropriate.
Respect peoples dignity, wishes and beliefs; involve them in shared decision-making and obtain their consent before undertaking procedures.
Promote a patient focused multidisciplinary approach to care in collaboration with carers, health care professionals and other agencies
To work according to the NMC Code of Professional Conduct and relevant professional guidelines as the Lead clinician for a defined group of patients.
Communication Core Dimension
Use communication skills in order to convey complex and sensitive information effectively to patients, carers and other staff, overcoming any barriers to understanding and providing support during distressing or emotional events.
Maintain professional boundaries and working relationships with patients and colleagues.
To act as patient/staff advocate through the application of ethical, legal and professional knowledge and skills.
Educate patients and their carer(s) as required where a need has been identified.

Job responsibilities
This position requires an autonomous specialist lead in developing, co-ordinating, evaluating and managing the Lymphoedema and Tissue Viability Service. The post holder will use advanced clinical expertise combined with managerial and leadership responsibilities to improve patient care, ensure efficient operations and promote positive teamwork.
This role will also promote clinical excellence through the provision of consistent specialised care, skilled communication, evidence based practice and continuous professional development of staff.
The TVN will work collaboratively to implement audit processes and systems within Pioneer for the purpose of promoting clinically effective and continuously improving care. The role will drive and help to continually embed Pioneer's values; putting Patients First ; effective Communication ; invest in Staff Development ; strive for Excellence ; and advance practice through Research .

Qualifications
Essential
1. Ability to evidence ongoing registration/revalidation as a Registered General Nurse
2. Ability to evidence relevant, recent and ongoing CPD/learning
3. Willingness to learn and achieve in the field of Tissue Viability and Lymphoedema
Experience
Essential
4. Able to demonstrate relevant experience of working at NHS Band 5 or above, or equivalent
5. Able to demonstrate experience in providing wound care
