Job summary Under the guidance of the Forget me not Unit Manager, the post holder will be autonomous in the planning and carrying out of specified ward-based group activities. The activity coordinator will lead on developing an activity programme, facilitating groups and individual interventions and activities to support patients in their goals. The post holder will work as a member of the multidisciplinary team, liaising closely with nursing and therapy staff to design person centred activities contributing positively to the emotional and psychological wellbeing of patients. The post holder is required to deal effectively with staff of all levels throughout the Trust. The post holder will work closely as part of the multidisciplinary team including Registered Nurses, Health care assistants, medical staff, Allied health professionals and volunteers. The post holder will be passionate about working with people living with dementia, and able to support meaningful activity whilst in hospital. The activities will be evidence based, and person centred. The post holder will have the ability to engage with patients with dementia and delirium and be able to utilise their effective communication skills to adapt their approach to support the individual. They will work to improve patient experience of hospital stays, and to maintain or rehabilitate patient's physical and cognitive skills. Main duties of the job To lead and encourage patients to join in meaningful activities whilst maintaining their independence, choice and dignity. To create a stimulating and varied programme of leisure activities and social events, for patients to enjoy. To work with each new patient to understand their likes/dislikes, abilities and needs. To liaise and consult with staff and relatives regarding the care of patients and particular activities that may be beneficial and stimulating.
