Job Summary
We have an exciting opportunity to recruit a Care Navigator to join our busy, friendly reception team. The successful applicant will be highly motivated, adaptable, and able to work under pressure.
Our Care Navigators work closely with our clinical team and wider colleagues in person and via email and tasks; therefore, excellent communication skills and strong IT skills are essential for the role. Experience working in a GP Surgery is essential. The position is permanent and subject to a 6-month probation period. Hours would be set, but flexibility is essential as there is a requirement to provide annual leave/sickness cover when needed.
The ideal candidate will be flexible to cover holidays and absences and undertake any administrative tasks to suit the needs of the practice while delivering an empathic and professional service to patients.

About Us
We are a friendly, supportive, and busy General Practice with approximately 7,300 patients. We have an established team with vast experience in dealing with patients and clinical queries. We have two very experienced GPs, Practice Nurses, on-site clinical Pharmacists, and many other Healthcare professionals. We are also part of the Primary Care Network (PCN) encompassing 8 local General Practices, which provides guidance and support and shares a variety of resources while looking out for the development of new services to improve patient care.

Job Responsibilities
* Answering calls and dealing with all aspects of patient-facing care including booking appointments, greeting, and directing patients.
* Relaying information and queries between patients and clinicians, ensuring clear and concise documentation.
* Care navigation (signposting patients to other appropriate services).
* Processing online consultations.
* Actioning and sending tasks to clinical team.
* Liaising with wider colleagues in primary and secondary care.
* Dealing with sensitive information while adhering to confidentiality policy.
* Full working knowledge of Emis for all functions required for the role.
* Book appointments on Emis and signpost to other services appropriately.
* Fill up appointments correctly for varied clinicians by signposting correctly.
* Input prescriptions accurately.
* Chase up daily DNAs and do tasks efficiently.
* Undertake other GP-related tasks such as patient registrations, referrals, and coding.
* Register new patients and educate them on the practice booking options.
* Identify and understand patient needs and requirements through appropriate questioning.
* Maintain high standards of patient confidentiality.
* Carry out all necessary general office duties.
* Maintain a clean, tidy, effective working area at all times.
Person Specification
Qualifications Essential
* GCSE grade A to C in English and Maths or equivalent.
Experience Essential
* Experience of working within a GP surgery.
* Experience of working on Docman.
* Experience of working on EMIS Web.
