We are looking for an experienced Relief Support Worker to provide person-centred support to people with a learning disability in Jarrow, South Tyneside. Your responsibilities will include: Providing intimate personal care, practical and emotional to people with a learning disability Identifying and accessing opportunities with the service users to support them to lead meaningful and fulfilling lifestyles Support individuals in the community with various activities. Including- shopping, disco & health appointments Collaborating with service users, families, and involved professionals to provide a consistent and coordinated service that meets the needs of service users Working as part of a team and alone in service user homes This is a fantastic service which enables great outcomes for the people we support. We are looking for Relief Support Workers who have the commitment and dedication to go the extra mile to enable people to achieve their personal goals. A minimum of twelve months experience of working with people with a learning disability is essential. A relevant professional qualification is desirable. You must be warm and able to work in a person-centred way.
