Job Overview
This is a 1-year fixed-term maternity leave cover post for our South Uttlesford PCN dietetic role. This position offers an exciting opportunity to gain experience within the South Uttlesford Primary Care Network (PCN), providing specialized dietetic resources for patients. There are 5 GP practices within the PCN with a registered population of approximately 51,500.
Main Duties of the Job
The post holder will be part of the PCN Primary Care Aligned Team (PACT) and will work closely with other multidisciplinary team members to support the management of patients with long-term conditions. Responsibilities include:
* Providing leadership and dietetic clinical expertise/advice in partnership with the multidisciplinary PACT.
* Integrating fully within the PCN team and becoming a significant source of expert dietetic advice.
* Clinically responsible for the dietetic assessment and treatment of patients with chronic diseases.
* Improving care for groups with specific health needs, such as diabetes, chronic pain, and menopause.
Person Specification
* HCPC registration
* Car owner/driver
* BSc Nutrition and Dietetics
* Ability to travel across sites
* Evidence of post-qualifying continued learning
