Job Title: Support Worker

Responsible to: Team Leader
Purpose of Role
To provide and promote person-centred care and support to vulnerable customers including people who have a range of mental health needs, learning and physical disabilities, behaviours that might challenge and require differing levels of support; to meet a range of complex social and health needs. To work effectively as part of a team, undertaking tasks as detailed within a person’s care plan, providing care and support to enable people to achieve their maximum level of independence through appropriate physical, emotional and psychological support including: intimate personal care, practical tasks, appropriate communication and community involvement.
Contributes to care plans and support planning and supports the communication process with families and health and social care professionals.
Responsibilities
* Practical Assistance
* Encourage and support people to be independent and to exercise choice and control over the support they receive.
* Use appropriate language and support customers to communicate in their preferred way (e.g., communication passport, makaton, BSL sign language, accessible information).
* Maximise the independence and functional ability of customers to enhance their quality of life, and develop and maintain life skills (e.g., encourage participation in recreational and social activities).
* Assist customers to develop or relearn basic life skills.
* Support customers in maintaining links with friends and other community groups.
* Support customers on outings, including accompanying them to hospital appointments.
* Provide travel training for customers.
* Demonstrate and communicate any additional aids (e.g., cutlery/crockery) to assist with eating and drinking.
* Deal with immediate emergency situations in accordance with prescribed procedures.
* Encourage and support community involvement, ensuring fair access and maximum benefit from available services.
* Provide assistance with household tasks (e.g., laundry, shopping, cleaning) and support with financial transactions.
* Personal Assistance
* Provide intimate personal care that respects the privacy and dignity of the person.
* Assist customers with daily living activities (e.g., getting up, dressing, washing, eating).
* Help customers with mobility issues and continence management.
* Support customers at the end of their life.
* Safeguard people’s human rights and ensure safety and well-being.
* Work as part of a team to provide flexible care and support based on individual care plans.
* Support medication administration in line with policy and procedures.
* Respect people’s culture, beliefs, and preferences in daily life and care services.
* Planning Requirements
* Record information into care plans and ensure timely reporting of needs.
* Engage with multi-agency partners to provide integrated services.
* Respect confidentiality and comply with data protection principles.
* Establish trust and confidence with customers and families.
* Participate in partnership work with professionals to review customer needs.
* Work independently and make sound judgments in emergencies.
* Participate in learning and development opportunities.
* Act in a professional manner in line with policies and procedures.
* Other Duties

Job Types: Full-time, Permanent
Pay: £24,027.00-£25,183.00 per year
