Job summary
The post holder will play a keyrole in supporting the Continuing Healthcare (CHC) Service. Working within theremit of the current National Framework for NHS Continuing Healthcare andNHS-funded Nursing Care, the post holder will deliver an effective andcompetent level of service that is consistently client-focused.
The post holder will assist and support in the facilitation ofmulti-disciplinary assessments and reviews, in line with standard operatingpolicy and procedures.
The post holder should possess basic knowledge of the NHS Framework forContinuing Healthcare and NHS Funded Nursing Care; along with local policy andpractice related to CHC.

Operational Support:
Highly motivated and enthusiastic individual toprovide and coordinate administrative services to include sourcing andcollating records, populating national tools for CHC, the preparation ofagendas and minutes, taking appropriate follow-up action as required.
The post holder will be required to review andcollate patient related documentation of individuals who are requesting NHSContinuing Health Care funding or require a Review of their care needs. Onoccasions, this may involve travelling to an individuals care setting.
The post holder will be required to identifyevidence of health care needs by following the guidelines as described in theNational Framework for NHS Continuing Healthcare and NHS-Funded Nursing Careusing various computer systems and liaising with other health and social care professionals.
Use up to date knowledge and evidence when undertaking care reviews andcare planning, advising and promoting health and well-being The post holderwill make person centered, evidence-based judgements in partnership with othersinvolved in the care process to ensure high quality care.
Recognise the limits of own competency and appropriately requestsupervision from a Registered Nurse to ensure the delivery of commissionedsafe, high-quality care.
Deliver and coordinate clinical reviews, personalised health budgets andcare planning to a caseload of patients as delegated by the Line Manager.
Ensure that all interventions are patientcentered; delivered compassionately, with dignity; whilst challenging andescalating poor practice / performance.
Report any concerns related to patients carefollowing case management to a Registered Nurse within the Continuing HealthcareTeam or Continuing Care Team for appropriate escalation to Care QualityCommission, Safeguarding services.
