An exciting opportunity has arisen for a Social Worker position within the Children with Disabilities Team. This role involves working with children, young people, and their families to determine social care needs and offering a holistic network of support, achieving positive outcomes and ensuring children and young people reach their potential.

The post holder will be responsible for planning and reviewing social care needs for children and young people with Special Educational Needs and Disabilities.
