Come join us and be part of our success. 2 x 24 hour posts The reablement service will sit along the Rapid Response Service within the wider Health and Social Care Services, and supports service users to achieve goals to get back to base line to become as independent as possible. You will be responsible for supporting people within their own home to admission and will promote the use of assistive technology to maximise independence. You will require the knowledge and understanding of Health & Social Care Organisations including experience of working to national care standards as well as a qualification at NVQ Level II in Care or a related subject or equivalent. Due to the nature of the work (delivering personal care) a GOQ is in place that only females apply As part of the role you will you need to have access to a car or equivalent mobility.
